The COREnology form also asks for your spouse or partner's name and email address, because each report requires its own address.
Please do not send confidential or time-sensitive information through the website. Submitting a form does not create an attorney-client relationship, and information sent before one exists may not be treated as confidential.
We do not require you to create an account, and we do not knowingly collect information from children.

How form submissions are processed
